Transitional or Intermediate Care
Programs targeted to provide individuals a place of overnight housing in a specialized residential facility or hospital setting for a limited period of time in order to enable and empower the individual to transition to a less restrictive community-based residential, own-home setting, or to a permanent family home.

Description |

Guardianship, Foster Care, Adoption, and Family Preservation | For the purpose of providing services for children who have been abused or neglected, including support services to families, providing a safe placement with kin or a foster family to children who have been removed from their homes, and subsidies to parents or guardians of children who have achieved permanency through adoption or guardianship. |
Intermediate Term Residential Services | For the purpose of providing intermediate term residential services to facilitate the safe and timely transition of residents back into the community. |
